Recently, when dumping and loading a database, I noticed that a FK in three
records of one table had lost their PK parent records.
What usually causes this "disconnect?"

If the records are truly lost, then the cause is usually an error within
the ASA server - technically you have a corrupt database with a broken
RI constraint.

There were several problems with early releases of 8.x related to this,
in particular if compressed B-tree indexes were used to enforce the FK
constraint (used if the maximum key length was greater than 9 bytes and
less than a proportion of the page size of the DB). As far as we are
aware, the latest EBFs of 8.x no longer are subject to such problems.
