Please explain what you are trying to do.
In the database painter you can select a table and then use the 'create
index' icon to create unique / duplicate indexes.

I'm not sure what you mean with 'in the books'. The syntax for creating
indexes is database dependent. For Oracle you would use:
create index index_name on table_name (col1, col2, ..)
