What would cause a large gap in an identity column? I have a gap from 58 to
500000000000002.

Rob Verschoor has good information on the identity gap issue called "A
better method for dealing with identity gaps". Read-up on it at his web
site:

Steve,

shutdown with nowait
server crashes
load a database dump

are the most common reasons for the jump in identity columns. There is a
document in the tech info library as well as numerous posts that cover this
issue. It is not a bug but a design issue that is addressed in 12.0.
